Details of the Incident: Provide a detailed account of the suspected fraud or misappropriation, including how it occurred and any relevant background information.

Date and Time: Specify when the incident took place.

Location: Indicate where the incident occurred.

Involved Parties:

Individuals or Entities Involved: List the names and roles of individuals or entities believed to be involved in the incident.

Evidence Attachment:

Upload documents, photos, videos, or other forms of evidence supporting your report.

Supported file types include PDF, JPG, PNG, and MP4.
